Out of State Physician Surrenders License

OLYMPIA, WA – Thomas J. Pulliam, MD (Lic. #60730018) voluntarily surrendered his license to practice after entering into a Stipulation to Informal Disposition with the Washington Medical Commission (WMC). This action was based on Dr. Pulliam entering into a Consent Order with the North Carolina Medical Board, where he resides. Dr. Pulliam later inactivated his North Carolina medical license.

The WMC alleged that Dr. Pulliam entered into a Consent Order with the North Carolina Medical Board reprimanding him for unprofessional conduct, including making a threat regarding a colleague.
